　　常年經營種田，再加上母親有病，不能干農活，有無其他收入，爺爺，奶奶年逾七十，又患有重病，在20__年奶奶因青光眼而做過手術，花了不少錢，由于爺爺是退伍軍人，在朝鮮戰爭中受過槍傷，在這一次手術，已經是第5次了，家里的負擔全落在父親身上，父親說再苦再累，也要供我上學。

　　我的思想情況：我積極上進，樹立正確的人生觀，有遠大的人生目標，通過自己的努力體現自我價值，在我的思想上認為雖然我現在還不是一名黨員，但我時刻在心里已經以一名黨員的標準嚴格要求自己了。

　　在生活上有條不紊，有良好的生活習慣，有著積極的人生態度，誠實守信，團結同學，與同學相處融洽。有良好的交際能力，在學校的交際范圍廣泛。宿舍溫馨和諧，舍友們相親相愛，團結互助。上學年以來五次被評為“文明宿舍”，還積極參加學校舉行的各類宿舍雅室大賽，其中在“我的心心小窩”雅室設計大賽中，榮獲了二等獎：在“溫馨標兵宿舍大賽”中獲得了三等獎。學院成立了宿舍委員會，作為一名學生干部，被老師任命為二棟女生宿舍的樓長，起到了模范到頭作用。為了盡量減少家庭的負擔，在休息時間和不影響學習的情況下，還兼職了一些校內外的勤工儉學工作，做過信息員、做過訪問員、做過禮儀。目前，我正在學校附近的麥當勞做兼職。雖然所得報酬有限，但是在一定程度上還是減輕了家里的經濟負擔，同時還學了一些書本上沒有的知識，這使我更加堅定了自己的人生信念。家庭的貧困只是暫時的，成績的好壞也已成過去，怎樣走好未來的路才是我自信和勇氣的見證，我一定會用青春的激情和毅力創造出更加絢爛的明天。鑒于我的家庭情況，以及自己一年來的在校表現，我認為自己符合此次國家勵志獎學金和國家貧困助學金的申請條件，特在此提出申請。

　　我是醫學技術系20__級醫學檢驗一班的___。光陰冉冉，歲月如歌，轉眼之間在大學第一年的光陰已經過去，回首去年九月懷著滿心的喜悅憧憬與滿腹的雄心壯志剛踏入陜西中醫學院的自己，一年的時光讓我成長了多，學會了許多，也進步了許多!過去的一年我始終保持著積極向上的心態，秉承“知行合一，嚴謹有為”的校訓，在良好的校園氛圍中，我時時以高標準來要求自己成為一名優秀的醫學生，并妥善處理好學習和工作兩者之間的關系，努力做到德、智、體全面發展。我認為自己在各方面均符合獎學金的評選條件，故提出該份申請書。現將本人基本情況介紹如下，作為各位領導的評審參考、

　　一、思想方面

　　大學是人生成長的重要階段，大學生是國家的希望與寶貴資源，所以我特別注重思想的進步和成熟，培養正確的人生觀、價值觀和世界觀，提高自己的思想政治覺悟。一直以來我都積極向黨組織靠攏，大一時我就努力想加入中國共產黨，并在五四評優時被評為“優秀共青團員”。

　　雖然現在我還未成為共產黨員，但是我時刻以共產黨員的標準來要求自己，時刻以先進正確的思想和理念武裝自己的頭腦，永記黨的宗旨，全心全意為人民服務。剛進入大一，我就加入了“青年志愿者協會”，在協會的一年中，在協會的組織下，參加了協會組織的各種義務勞動，身為志愿者的一員，我感到萬分的榮幸!在平時學習、工作和生活中我更是嚴格要求自己，力爭做到工作中任勞任怨，生活中艱苦樸素，時時處處在同學中起表率作用。大一一年，我積極參加了班級系里組織的各種活動，并在一些活動中取得了不錯的成績!

　　二、學習方面

　　在兩個學期的學習中，我始終牢記、學習是學生的第一要務。在過去的一年中，我刻苦學習，努力務實，抓緊分分秒秒的'時間努力學習知識，同時博覽群書，拓寬知識面，我喜歡讀各種不同的書了解不同的領域，在這個過程中開拓了我的視野。“寶劍鋒從磨礪出，梅花香自苦寒來。”在老師、同學們的指導和幫助下，我取得了比較理想的成績、在第一學年的考試中，總成績排名班級第一!我相信一分耕耘一分收獲，只有付出努力才會達到自己想要的高度!

　　此外在大一期間我參加了計算機二級的培訓課，努力學好計算機，并在今年九月份參加了考試，我相信我一定可以通過此次考!看著自己用汗水換來的成績，我知道自己在不斷進步，為自己感到欣慰的同時我知道我還要更加不懈的努力，以取得更大的進步。

　　三、工作方面

　　入學初期，我擔任了檢驗一班學習委員，并在第二學期的競選中再次當選!在工作中，我堅持認真負責的態度，勤勤懇懇的做好我的本職工作。作為學習委員，我在學習上嚴格要求自己為大家做好表率作用，努力為同學們營造一個良好的學習氛圍，平時也會督促大家按時完成作業和學習任務!平時學習之余，我還組織了一些類似于辯論賽，茶話會之類的班級活動，鍛煉了同學們的能力也增強了同學之間的感情，促進了班級凝聚力!在這一年的工作中，我積極很好的與各個代課老師配合教學工作，在每個老師身上都學到了很多課本上學不到的東西，并起到了師生之間的橋梁作用。前不久，我作為學姐參加了大一學生的“學習生活經驗交流會”，為大一新生撥開迷茫迎接嶄新的大學生活，會后好多同學找我談心，我都耐心的給她們我的建議。在大二這一年，我將繼續擔任學習委員這一職務，并且以飽滿的熱情投身到接下來工作中去，再接再厲!

　　四、生活方面

　　受父母的影響，生活上，我艱苦樸素，嚴以律己寬以待人，在生活中建立了良好的人際關系，獲得了大家的一致公認!“沒有最好，只有更好!”是我的人生信條!在未來的生活中，我將以百倍的信心和萬分的努力去迎接更大的挑戰，用辛勤的汗水和默默的耕耘譜寫更美好的明天!
